Articular cartilage defects are seen in a small percentage of knees that come up for arthroscopy. They may be associated with ACL insufficient knees. They can progress to osteoarthritis in the long term. They cannot heal naturally. The choice of a surgical procedure to heal these defects depends upon the demands of the patient and the size of the defect. Small defects in low demand patients can be treated by micro fracture, periosteal flap or osteochondral transplant. Large defects in high demand individuals can be treated by autologous cartilage cell transplant. In this procedure cartilage cells are harvested from the knee arthroscopically. These are cultured in the laboratory. The regenerated cartilage cells are transplanted into the defect by another open operation under a perisosteal flap, which is sutured to the edges of the defect. Post operatively the patient has to remain non-weight bearing for six weeks and undergoes continues passive motion therapy.
