Pain less knee replacement
Patients' anxieties
Major surgery like TKR,THR are a physiological challenge to the body and involve some insult to the tissues by cutting soft tissues and bones, cauterising blood vessels etc. These damaged tissues can liberate pain producing substances which trigger post operative pain. Pain is unpleasant and delays recovery.
Humanitarian pain relief
The two objectives of post op analgesia are humanitarian pain relief and faciltate rehabilitation and return to function. My patients will testify that they have no pain in the post op period and this feel good factor continues into the early recovery phase. Expert anesthetic and analgesic management facilitates this hitherto unpleasant part of the operation. Hence they require lesser amount of narcotics. They can cooperate with active movements of the knee to enable quicker return of knee movements.
Criteria of freedom from pain.
The patient should be able to do a straight leg raise as shown in the photo. He should be able to bend the knee and keep it straight.
Modalities of perioperative pain
Regioanal anesthesia, nerve blocks, articular injection, Cryotherapy are all useful to prevent and reduce pain and are routinely employed in my practice.

Continuous femoral nerve block administered for TKR
In the accompanying picture, a catheter has been placed for administration of analgesia intra operatively and post operatively. This regional analgesia decreases the requirement for opioids post operatively. Side effects like vomitting, nausea, retention of urine, constipation are avoided.
